If you need support for your patient portal account or are having trouble accessing your medical record online, please contact your physician’s office directly.
Your support request will be submitted via email. Please do not include any Protected Health Information (PHI). This includes any information that personally identifies the patient, such as name or date of birth. If you need to reference a specific referral, please do so using only the Treatspace Referral ID.
